Updated Comprehensive Informed Consent Forms: Immunizations & Vaccines

Dear Massachusetts Legislators,

Brookline consumers know what ingredients are in their foods, and need similar information about their immunizations and vaccines. We therefore request:

1. All vaccine / immunization ingredients be listed on official Informed Consent forms for immunizations.

2. List EPA Daily Limits on Vaccine Ingredients listed by the EPA as toxins.

3. List the FDAs required studies on the vaccines Safety and Efficacy and have them on file for public examination.

4. List independent Parent / Consumer funded studies documenting the risks of vaccines and have these on file for public examination.

5. Independent Parent groups may place studies on file.

6. Honor the Childrens Vaccine Injury Act of 1984, by informing the public about the CDCs vaccine adverse event reporting system (VAERS).

7. Provide CDC Statistics Revealing Vaccine Injuries & Deaths

8. Provide CDC Graphs reflecting Vaccine Injury Awards

9. Provide the CDCs projected figures for vaccine injuries on recommended vaccine.

10. Provide information on government liability for vaccine injuries.

11. Mail informed Consent forms with Exemption Forms to parents of children in Public Schools
